Consultation Fee
There is a non-refundable $50 consultation fee. This fee can be applied to services received the day of your appointment. However, if you do not attend your appointment this fee will not be refunded.
Intake Forms and Appointment Confirmation:
Intake forms are automatically emailed once your appointment is scheduled and must be completed before your appointment. To ensure a smooth appointment, please complete the intake forms at least 24 hours prior to your scheduled appointment. Failure to do so may result in rescheduling of your appointment.
Refund Policy
Due to the nature of the services that we provide, under NO circumstances are refunds allowed for any services or products purchased at Six62 Aesthetics and Wellness.
HIPAA NOTICE OF PRIVACY PRACTICES
The HIPAA notice describes how medical information about you may be used and disclosed and how you can obtain access to this information. Please review it carefully.
No Call/No Show Policy
Failure to show up to your scheduled appointment without notifying us will result in a $50 fee. This fee must be paid before confirming future appointments.
Late Arrival:
If you are more than 10 minutes late for your scheduled appointment, your appointment will be canceled.
Cancellation:
Appointments must be canceled within 24 hours of the appointment date and time, failure to do so will result in a cancellation fee of $50.
Card on File:
All clients are required to have a valid card on file.
